LifeSpan Home Health Care is an Indiana state-licensed home health agency that is conveniently located on the campus of Bell Trace Senior Living Community. The professional staff at LifeSpan will meet with you and your family to discuss an individual plan of care to meet specific medical needs. The goal of LifeSpan Home Health services is to help you remain independent.

LifeSpan can provide the following services:

• Comprehensive assessments
• Home safety assessments
• Medication management
• Medication supervision
• Continence care
• Respite care for short-term needs
• Compassionate short-term or long-term care depending on needs
• Daily, occasional or 24-hour care, from light support to comprehensive care
